Correlation Survey Template

Correlation studies need clean paired answers on the same scale. This template asks for two named 1 to 5 measures so you can look for a relationship later in a spreadsheet.

Use the same 1 to 5 scale for both constructs. Rename the labels to your variables (for example sleep quality and focus) before you share the link.
