Use the same 1 to 5 scale for both constructs. Rename the labels to your variables (for example sleep quality and focus) before you share the link.
Share one Formms link. Export CSV when you are ready to chart or run a correlation (Pearson on 1 to 5 is a first pass; Spearman fits ordinal ranks).
