Paste a Google Form share link and download a PNG you can print or drop on a slide. Generation runs in your browser. The link is not uploaded or stored.

1. Copy your form link
In Google Forms, copy the responder link. A forms.gle short URL or a full docs.google.com/forms URL both work. Short links make a less dense code, which helps on small stickers.
2. Paste it and generate
Open the Google Form to QR Code tool. Paste the URL. Click Generate QR code. You can pick a style next to the preview if you want.

3. Download the PNG
Click Download PNG. Test-scan it with a phone before you order a print run. Confirm the form opens, and that anyone with the link can respond if that is what you intended.

The PNG is a static code. It keeps working as long as that form URL still opens. Editing questions on the same form does not need a new code. Generate a new one only if the public link changes.
Formms does not count scans. You will see replies in Google Forms. For a SurveyMonkey collector instead, use SurveyMonkey to QR Code.
