Upload a PDF and Formms rebuilds the questions as an online form. You can fill the preview without an account. Save the form if you want a shareable link and a place to collect answers.
The tool is at form.ms/tools/pdf-to-form-converter. This walkthrough uses a one-page Maya and Ben wedding RSVP.
1. Open the converter
Go to the PDF to Form Converter. You will see a drop zone for a PDF up to 8 MB.

2. Upload the RSVP PDF
The sample card asks for guest name, whether they are coming, a meal choice, and optional dietary notes.

Click Choose a PDF or drop the file on the zone. Born-digital PDFs and scans both work. The file is sent to Formms, read once, and is not kept.
3. Preview the rebuilt form
When conversion finishes, the same questions appear as a live form. Guest name is a short text field. Attending and meal choice are multiple choice. Dietary notes is a longer text field. Fill it in to check. You do not need an account. The draft is deleted after 7 days unless you save it.

4. Save if you want a link
Click Save this form free to keep it in a Formms account. Then you can edit questions, share a form.ms link or QR, and collect answers.
