What is Formms?

Formms is a free AI form builder for iPhone, Android, and the web. You describe what you need in plain language, Formms drafts the questions, and you send people a short form.ms link, a QR code, or an embed. Build at my.form.ms or in the iOS and Android apps. Once you sign in, the same forms show up in all three. People who fill in a form never need an account or the app.

Make a form

On the web, click + New form. On iPhone or Android, tap + Create a form.

From there you can create a form with AI, start from scratch, or import a form. AI is the usual starting point: you say what the form is for, who will fill it in, and what you need to collect. From scratch is Start blank on the web and Scratch on the phone. Import needs a public Google Form, Typeform, or Tally link. You can also attach a PDF or a photo of a paper form and Formms will turn that into questions.

Question types

Every form has a title, a description, and questions under that. Click a field to change the wording. Drag to change the order.

The picker has 16 types. Most of them collect an answer. Statement is the exception: it is a heading and body you can drop in as a section, not a field people fill in.

• Short text: a single line, like a company name or a short answer

• Name: a full name

• Long text: a paragraph, like comments or a cover note

• Email: a validated email address

• Phone: a phone number

• Number: any number, such as age, years, or a budget

• Quantity: a count with minus and plus

• Single choice: pick one option

• Multiple choice: pick several

• Dropdown: pick one from a list

• Rating: 1 to 5 stars

• Linear scale: 0 to 10

• Date: a calendar

• Yes / No: two options

• File upload: a resume, photo, or PDF

• Statement: a heading and body, not an answer

Payments and signatures are not in the picker. The AI helper can still mention them. Skip those and use the types above.

Required questions show a red asterisk. Optional ones do not. If you want to rewrite several questions at once instead of editing one by one, use Chat to Edit. See Add questions.

Share

Nothing is public until you publish. A Draft has no form.ms URL. If you open Share before that, you will see Ready to go live, and you need at least one question before you can publish. The first time you click Publish, Formms opens Share for you. After that, Publish leaves the toolbar and Preview stays.

Once the form is live you get:

• A short public URL, https://form.ms/xxxxx

• A QR code you can download and print

• Website embeds: Standard, Popup, or Full page

• A WordPress iframe snippet

• A Tag Manager option

Answers

Answers stay on the owner account. Open Responses for a summary, a list of every submission, and Export CSV if you need a spreadsheet. There is no live Google Sheets sync.

The free plan does not cap how many responses you can collect. Leave Limit responses empty (No limit) unless you want a cap. You can stop taking answers by turning off Accept responses or by setting a close date.

After someone submits, they see a thank you message. That is one message, not a separate page. If the form is closed, they see the closed message instead. You can get a push on iPhone or Android when a response comes in, and an email if you turn that on, for one form or for the whole account.
